Our Support Coordinators are here to work with you implement choice and control in all your decisions and effectively manage and track your NDIS Plan. Our team will take the time to get to know you, those close to you and your chosen services to help implement and maintain the right solution for you.
A Support Coordinator’s role is to:
– Link you with your community and government services
– Tracking spending
– Personal Advocate
– Care Team Liaison
– Help you to understand your plan and how to best utilize it
– Keep you up to date with changes or new services
– Collaborating with all your services
– Work with you to achieve your NDIS Goals
